§ 28-29-14 Forms prescribed and furnished.– The director shall prescribe the form of a notice informing employees of theirprivilege under chapters 29 – 38 of this title, and the notice shall beincorporated in the notice prescribed in this chapter. Blank forms of electionand withdrawal, as well as summaries as provided by § 28-29-13, shall befurnished by the director, on request.
