§ 90‑118.1. Contents of original license.
The original license granted by the North Carolina State Board ofExaminers in Optometry shall bear a serial number, the full name of theapplicant, the date of issuance and shall be signed by the president and a majorityof the members of the said Board and attested by the seal of said Board and thesecretary thereof. The certificate of renewal of license shall bear a serialnumber which need not be the serial number of the original license issued, thefull name of the applicant and the date of issuance.
