Insured to be furnished application for certificate and annualstatement, when.
376.930. At the time of or prior to the execution of a lifecare contract and the transfer of any money or other property toa provider pursuant thereto, the provider shall deliver to theperson with whom the life care contract is entered into a copy ofthe provider's application for a certificate of authority or mostrecent annual statement as prescribed by section 376.915,whichever is most recent. Thereafter, the provider shallannually deliver a copy of the annual statement to each residentof the facility upon request within ten days after such annualstatement is filed with the department.
