355.016. 1. The secretary of state may prescribe and furnish onrequest, forms for:
(1) A foreign corporation's application for a certificate ofauthority to transact business in this state;
(2) A foreign corporation's application for a certificate ofwithdrawal; and
(3) The corporate registration report.
If the secretary of state so requires, use of these forms is mandatory.
2. The secretary of state may prescribe and furnish on request formsfor other documents required or permitted to be filed by this chapter buttheir use is not mandatory.
