345.051. 1. Every person licensed or registered pursuant to theprovisions of sections 345.010 to 345.080 shall renew the license on orbefore the renewal date. Such renewal date shall be determined by theboard. The application shall be made on a form furnished by the board.The application shall include, but not be limited to, disclosure of theapplicant's full name and the applicant's office and residence addressesand the date and number of the applicant's license, all final disciplinaryactions taken against the applicant by any speech-language-hearingassociation or society, state, territory, federal agency or country andinformation concerning the applicant's current physical and mental fitnessto practice as a speech-language pathologist or audiologist.
2. A blank form for application for license renewal shall be mailedto each person licensed in this state at the person's last known office orresidence address. The failure to mail the form of application or thefailure to receive it does not, however, relieve any person of the duty torenew the license and pay the fee required by sections 345.010 to 345.080for failure to renew the license.
3. An applicant for renewal of a license pursuant to this sectionshall:
(1) Submit an amount established by the board; and
(2) Meet any other requirements the board establishes as conditionsfor license renewal, including the demonstration of continued competence topractice the profession for which the license is issued. A requirement ofcontinued competence may include, but is not limited to, continuingeducation, examination, self-evaluation, peer review, performance appraisalor practical simulation.
4. If a license is suspended pursuant to section 345.065, the licenseexpires on the expiration date as established by the board for all licensesissued pursuant to sections 345.010 to 345.080. Such license may berenewed but does not entitle the licensee to engage in the licensedactivity or in any other conduct or activity which violates the order ofjudgment by which the license was suspended until such license has beenreinstated.
5. If a license is revoked on disciplinary grounds pursuant tosection 345.065, the license expires on the expiration date as establishedby the board for all licenses issued pursuant to sections 345.010 to345.080. Such license may not be renewed. If a license is reinstatedafter its expiration, the licensee, as a condition of reinstatement, shallpay a reinstatement fee that is equal to the renewal fee in effect on thelast regular renewal date immediately preceding the date of reinstatementplus any late fee established by the board.
