Certificate of registration--notice to renew--fees--display ofcertificate, requirements.
334.570. 1. Every person licensed under sections 334.500 to 334.620shall, on or before the registration renewal date, apply to the board for acertificate of registration for the ensuing licensing period. Theapplication shall be made under oath on a form furnished to the applicantby the board. The application shall include, but not be limited to,disclosure of the following:
(1) The applicant's full name;
(2) The applicant's office address or addresses and telephone numberor numbers;
(3) The applicant's home address and telephone number;
(4) The date and number of the applicant's license;
(5) All final disciplinary actions taken against the applicant by anyprofessional association or society, licensed hospital or medical staff ofa hospital, physical therapy facility, state, territory, federal agency orcounty; and
(6) Information concerning the applicant's current physical andmental fitness to practice his or her profession.
The applicant may be required to successfully complete a test administeredby the board on the laws and rules related to the practice of physicaltherapy. The test process, dates, and passing scores shall be establishedby the board by rule.
2. A notice for application for registration shall be made availableto each person licensed in this state. The failure to receive the noticedoes not, however, relieve any person of the duty to register and pay thefee required by sections 334.500 to 334.620 nor exempt such person from thepenalties provided by sections 334.500 to 334.620 for failure to register.
3. If a physical therapist does not renew such license for twoconsecutive renewal periods, such license shall be deemed void.
4. Each applicant for registration shall accompany the applicationfor registration with a registration fee to be paid to the director ofrevenue for the licensing period for which registration is sought.
5. If the application is filed and the fee paid after theregistration renewal date, a delinquent fee shall be paid; except that,whenever in the opinion of the board the applicant's failure to register iscaused by extenuating circumstances including illness of the applicant, asdefined by rule, the delinquent fee may be waived by the board.
6. Upon application and submission by such person of evidencesatisfactory to the board that such person is licensed to practice in thisstate and upon the payment of fees required to be paid by this chapter, theboard shall issue to such person a certificate of registration. Thecertificate of registration shall contain the name of the person to whom itis issued and his or her office address, the expiration date, and thenumber of the license to practice.
7. Upon receiving such certificate, every person shall cause thecertificate to be readily available or conspicuously displayed at all timesin every practice location maintained by such person in the state. If thelicensee maintains more than one practice location in this state, the boardshall, without additional fee, issue to such licensee duplicatecertificates of registration for each practice location so maintained. Ifany licensee changes practice locations during the period for which anycertificate of registration has been issued, the licensee shall, withinfifteen days thereafter, notify the board of such change and the boardshall issue to the licensee, without additional fee, a new registrationcertificate showing the new location.
8. Whenever any new license is granted to any physical therapist orphysical therapist assistant under the provisions of this chapter, theboard shall, upon application therefor*, issue to such physical therapistor physical therapist assistant a certificate of registration covering aperiod from the date of the issuance of the license to the next renewaldate without the payment of any registration fee.
