Licensure, application, fee, contents--license duration--renewal--lostor stolen license, replacement of.
334.404. 1. Each person desiring a license pursuant to sections334.400 to 334.430 shall make application to the board upon such forms andin such manner as may be prescribed by the board and shall pay the requiredapplication fee as set by the board. The application fee shall cover thecost of issuing the license and shall not be refundable. Each applicationshall contain a statement that it is made under oath or affirmation andthat its representations are true and correct to the best knowledge andbelief of the person signing the application, subject to the penalties ofmaking a false declaration or affidavit. Such application shall includeproof of certification from the National Commission on Certification ofAnesthesiologist Assistants or its successor, date of the certification,any identification numbers, and any other information necessary for theboard to verify the certification.
2. The board, upon approval of the application from an applicant,shall issue a license to such applicant.
3. A license is valid for two years from the date it is issued andmay be renewed biennially by filing an application for renewal with theboard and paying the required renewal fee as set by the board.
4. A blank form for application for renewal of licensure shall bemailed to each person licensed in this state at his or her last knownoffice or residence address.
5. A new license to replace any license lost, destroyed, or mutilatedmay be issued to any applicant, subject to rules and regulations issued bythe board upon the payment of a reasonable fee.
