333.031. Each application for a license to practice eitherembalming or funeral directing shall be in writing, addressed tothe board, on forms prescribed, verified and shall contain suchinformation as is required by the board. The application shallinclude a statement that it is made under oath or affirmation andthat its representations are true and correct to the bestknowledge and belief of the person signing same, subject to thepenalties of making a false affidavit or declaration. Eachapplication shall be accompanied by an embalming fee or funeraldirecting fee. Any applicant for both a license to practiceembalming and to practice funeral directing shall pay both fees.If any applicant for a license to practice embalming or funeraldirecting fails to pass the examination given by the board, hemay be given other examinations upon payment of a reexaminationfee.
