Applications for registration, form--penalties.
324.415. Applications for registration as a registered interiordesigner shall be typewritten on forms prescribed by the council andfurnished to the applicant. The application shall contain the applicant'sstatements showing the applicant's education, experience, results ofprevious interior design certification, registration or licensingexaminations, if any, and such other pertinent information as the councilmay require, or architect's registration number and such other pertinentinformation as the council may require. Each application shall contain astatement that is made under oath or affirmation and that therepresentations are true and correct to the best knowledge and belief ofthe person signing the application. The person shall be subject to thepenalties for making a false affidavit or declaration and shall beaccompanied by the required fee.
