Application to be submitted, contents--fee.
301.219. Application for a license shall be submitted biennially andshall be made on the form the department prescribes, containing the name ofthe applicant, the address where business is to be conducted, the kind ofbusiness, enumerated in section 301.218 to be conducted, the residenceaddress of the applicant if an individual, the names and residenceaddresses of the partners of the applicant if a partnership, the names andresidence addresses of the principal officers of the applicant and thestate of its incorporation, if a corporation. The application shall beverified by the oath or affirmation of the applicant, if the applicant is apartnership or a corporation, by a partner or officer of the applicant andshall be accompanied by a fee of one hundred thirty dollars every two yearsfor each kind of business required to be licensed under subdivision (1),(2), (3), or (4) of subsection 1 of section 301.218. If the applicantconducts business at different locations, a separate application, licenseand one hundred thirty dollar fee shall be required for each location. Thedirector may stagger the expiration dates to equalize the workload.
