Contents and form of work certificates.
294.054. 1. The work certificate shall show
(1) The age, sex, place and date of birth and place ofresidence of the child;
(2) The name and place of residence of the child's parent,guardian or custodian;
(3) The name and address of the employer; and
(4) The nature of the employment for which the workcertificate is issued.
2. Work certificates authorizing the employment of a childduring nonschool hours shall be of a form and color distinct fromthose authorizing full time employment.
