§ 18-212. Health officer records.
 

(a)  Record required.- The health officer shall keep a record of each report that is submitted to the health officer under § 18-201 of this subtitle. 

(b)  Form and contents.- The record shall: 

(1) Be on the form that the Secretary provides; 

(2) State the date of the report; 

(3) State the name of the physician who submits the report; 

(4) State the disease identified in the report; 

(5) State the name and residence address of the patient named in the report; and 

(6) State the actions taken to prevent the spread of the disease.
