Any provider licensed or certified under the laws of the Commonwealth whose license or <br>certificate to practice is suspended, revoked, limited, or otherwise restricted shall have <br>that fact reported to the medical assistance program by the respective licensure or <br>regulatory board or agency within five (5) working days of the action.
