321.182 - APPLICATION.

        321.182  APPLICATION.         Every applicant for a driver's license shall do all of the      following:         1. a.  Make application on a form provided by the department      which shall include the applicant's full name, signature, current      mailing address, current residential address, date of birth, social      security number, and physical description including sex, height, and      eye color.  The application may contain other information the      department may require by rule.  Pursuant to procedures established      by the department and for an applicant who is a foreign national      temporarily present in this state, the department may waive the      requirement that the application include the applicant's social      security number.         b.  A licensee shall notify the department when the licensee's      mailing address changes and provide the new address within thirty      days of obtaining the new address.  The application provided by the      department shall include a statement for the applicant to sign that      acknowledges the applicant's knowledge of the requirement to notify      the department of a mailing address change.  The penalty under      section 321.482 shall not apply to a licensee's failure to notify the      department of such an address change.         2.  Surrender all other driver's licenses and nonoperator's      identification cards.         3.  Certify that the applicant has no other driver's license and      certify that the applicant is a resident of this state as provided in      section 321.1A.  However, certification of residency is not required      for an applicant for a nonresident commercial driver's license who is      a foreign national temporarily present in this state, as determined      by the department.         4.  Certify that the applicant is not currently subject to      suspension, revocation, or cancellation of any driver's license and      has committed no offense and has not acted in a manner which either      alone or with previous actions or offenses could result in      suspension, revocation, or cancellation of any driver's license.
