INSTRUCTIONS : COMMUNITY SERVICE FORM


A community service form is a document that is used to show completion of community service.  Usually, community service is mandated for criminal or statutory violations instead of jail or probation.  This form is used to prove community service obligations were met.  

In this case, we are using the community service form used by Family Drug Treatment Court in the state of Washington.  Many different examples of community service forms can be found online or through the court in which community service was given.   

The first step in completing the form is to enter the participant's name and the amount of total hours of community service they are ordered to serve.  


1. A deadline should be provided, informing the participant of when they must turn their community service form in. Fill in this time on each form submitted.


2.  If a drug court date is scheduled, it should be written on the community service form.  


3. The next step is to fill out the grid indicating the time spent on community service.


4. Provide the name of the agency where the community service was completed, their phone number, your supervisor's contact name, and the date in which you competed the community service. 


5. The agency must sign each instance of community service, providing your sign in time and sign out time. If they have any additional comments, they will write them in the comments section.


6. The participant must then sign and date the form, indicating the town in which they signed it.  


7. Additional pages may be necessary if more community service time was completed than can fit on the grid provided.  If you file additional sheets, ensure that your provide the required information on each individual sheet.  


8. Submit the form to the appropriate case manager and keep a copy for your records.
