INSTRUCTIONS: ARKANSAS REPORTING FIRM AUTHORIZATION / POWER OF ATTORNEY
To designate a firm or individual with power of attorney in dealing with the Arkansas Department of Finance and Administration, file the form discussed in this article. It can be obtained from the website maintained by the Arkansas Department of Finance and Administration.
Arkansas Reporting Firm Authorization / Power Of Attorney Step 1: Section 1 concerns the taxpayer. In the first box, enter their name and complete home address.
Arkansas Reporting Firm Authorization / Power Of Attorney Step 2: In the second box, enter their Social Security number, as well as that of their spouse if applicable.
Arkansas Reporting Firm Authorization / Power Of Attorney Step 3: In the third box, enter the taxpayer's sales tax permit number.

Arkansas Reporting Firm Authorization / Power Of Attorney Step 8: Another two boxes are provided if you wish to appoint another representative.
Arkansas Reporting Firm Authorization / Power Of Attorney Step 9: Section 3 concerns the types of tax matters you are authorizing your representatives to have power of attorney over. In the column on the left, enter the type of taxes they are authorized to have power of attorney over.
Arkansas Reporting Firm Authorization / Power Of Attorney Step 10: In the column on the right, enter the years or periods for each tax type your representatives are entitled to have power of attorney over.
Arkansas Reporting Firm Authorization / Power Of Attorney Step 11: In Section 4, you may list any specific additions or deletions to your representatives' power of attorney as explained here.
