INSTRUCTIONS: REQUEST FOR COPIES OF ARKANSAS TAX RETURNS AND W-2(S) (Form AR4506)

To request a copy of your Arkansas tax return or W-2 forms, file a form AR4506. This document can be obtained from the website maintained by the Arkansas Department of Finance and Administration.

Request For Copies Of Arkansas Tax Returns And W-2(s) AR4506 Step 1: In the first blank box, enter the primary name on the return.

Request For Copies Of Arkansas Tax Returns And W-2(s) AR4506 Step 2: In the second blank box, enter the Social Security number, federal employer identification number or ID number associated with the primary name.

Request For Copies Of Arkansas Tax Returns And W-2(s) AR4506 Step 3: In the third blank box, enter the secondary name on the return, if applicable.

Request For Copies Of Arkansas Tax Returns And W-2(s) AR4506 Step 4: In the fourth blank box, enter the Social Security number or ID number associated with the secondary name, if applicable.

Request For Copies Of Arkansas Tax Returns And W-2(s) AR4506 Step 7: In the seventh blank box, enter the tax year or years from which you are requesting copies of your tax returns.

Request For Copies Of Arkansas Tax Returns And W-2(s) AR4506 Step 8: In the eighth blank box, enter the year or years from which you are requesting copies of your W-2s.

Request For Copies Of Arkansas Tax Returns And W-2(s) AR4506 Step 9: On the first blank line at the bottom of the page, enter your signature and the date.

Request For Copies Of Arkansas Tax Returns And W-2(s) AR4506 Step 10: On the second blank line, enter your title if the primary name is a partnership or a trust.

Request For Copies Of Arkansas Tax Returns And W-2(s) AR4506 Step 11: On the third blank line, the secondary signature, if applicable, should be entered, along with the date. For each copy you request, a $2 fee must be paid. Submit payment via check or money order along with this form.
