INSTRUCTIONS: ARIZONA TRANSFEROR NOTICE OF TRANSFER OF MOTION PICTURE CREDITS (Form 334-O)

Within 30 days of the sale or transfer of an Arizona motion picture credit, a form 334-O must be filed with the state Department of Revenue by the transferor. This document can be obtained from the website of the Arizona Department of Revenue.

Arizona Transferor Notice Of Transfer Of Motion Picture Credits 334-O Step 1: Part A concerns the transferor. Provide the transferor's name, tax identification number, number and street or PO box, city or town, state, and zip code, as well as the name of an authorized contact person and their telephone number.

Arizona Transferor Notice Of Transfer Of Motion Picture Credits 334-O Step 2: Part B concerns the motion picture production company. Enter their name, tax identification number, number and street or PO box, city or town, state and zip code, as well as the name of an authorized contact person and their telephone number.

Arizona Transferor Notice Of Transfer Of Motion Picture Credits 334-O Step 3: On line 1 of Section C, enter the credit postapproval number.

Arizona Transferor Notice Of Transfer Of Motion Picture Credits 334-O Step 4: Skip to Part D on the second page. For each transferee, enter their name, number and street, city or town, state and zip code, as well as the name of their authorized contact person and their telephone number. On the right, give their tax identification number and their amount of the credit.

Arizona Transferor Notice Of Transfer Of Motion Picture Credits 334-O Step 5: Add the total credits documented on lines 1 through 4 for each transferee. Enter the total on line 5.

Arizona Transferor Notice Of Transfer Of Motion Picture Credits 334-O Step 6: Transfer the value from line 5 of Part D to line 2 of Part C.

Arizona Transferor Notice Of Transfer Of Motion Picture Credits 334-O Step 7: Enter the date of the transfer on line 3 of Part C.

Arizona Transferor Notice Of Transfer Of Motion Picture Credits 334-O Step 8: Enter the transfer credit balance before the transfer on line 4 of Part C.

Arizona Transferor Notice Of Transfer Of Motion Picture Credits 334-O Step 9: Enter the transfer credit balance after the transfer on line 5 of Part C. The transferor should sign and print their name at the bottom of the first page.
