INSTRUCTIONS: ALABAMA APPLICATION TO PREPARE A NEW BIRTH CERTIFICATE AFTER ADOPTION (Form ADPH-HS 88)

To request that an adopted child be issued a new birth certificate, a form ADPH-HS 88 should be filed. This document can be obtained from the website of the Alabama Department of Public Health.

Alabama Application To Prepare A New Birth Certificate After Adoption ADPH-HS 88 Step 6: If requesting a certificate for a child born in Alabama, enter the number of copies being requested and the fee enclosed. The first copy will cost $25, with an additional $6 charged for each subsequent copy.
